Description

Designed and developed the Alzheimer's Disease Patient Database System for Alzheimer's Clinic. The system is for tracking patient medical history records including various diagnostic test results. It is used to analyze and produce statistical reports. The critical historic data was imported from main frame based legacy system. The new system was developed in MS-Access.

Goal

To provide the various clinical personnel with an easy to use and powerful tool to enter and query their data so that they can perform statistical and trend analysis.
For example, the administration personnel had the need to enter patient information and be able to quickly retrieve patient statistical information.

Approach

We worked collaboratively with the doctors and administration to take their methodologies for statistical analysis of patient data and transferred those to a reporting function in a database application.
Some of the application capabilities included:
Search Tools: Search patients by last name, first name, full name or MRN. Refine search with match by first name or last name partial values.

Maintain Patient demographics including details on registration, patient and next of kin address, refering physician, family physician and films.

Maintain and Track unlimited Patient visits.

Maintain and Track patients per visit treatments in areas of Genetics, Psychiatry, Speech, Psychology, Social Work, Medical and Neuropathology.

Maintain Patients Genetics including family history, dementia, down syndrome, psychiatric illness, alcoholism.

Track Patients per visit Medical details in areas of blood work, radiology and physical condition details.

Track Patients per visit Social Work details including Caregiver Burden Scale questionnaire, Disability Assessment for Dementia scoring and Long term care.

Track Deceased Patients Neuropathology details

Scoring: Score Patient characteristics per visit in Psychiatry, Speech and Nueropsychology Test Battery Profile.

• Psychiatry characteristics including Paranoid and Delusional Ideation, Hallucinations, Activity disturbance, Aggressivity, Diurnal rhythm disturbance, Affective disturbance, Anxieties and Phobias.

• Speech characteristics including Story retelling narrative, Repetition, Sentence comprehension, Reading comprehension, Token test, Boston naming, Hearing screening and Family report.

• Neuropsychology Test Battery Profile (Raw score, Perentile, Transformed, Z score) in test areas WaisR/WAISIII, Oral word association, Boston naming, Benton visual, Rey-osterrieth, Wechsler memory scale R/III, RAVLT/CVLT, Trail making, Wisconsin card sorting, Categories test, Stroop test, Ravens coloured matrices, Hooper visual orientation, Finger tapping, Luria-Nebraska designs, Buschke cued recall, Present functioning questionnaire, Geriatric depression scale and Behavioural rating score.

This scoring tool allows the clinic to establish the scoring criteria for each pull down field and uses complex programmed algorithms to determine the total scores for each test type.

Results

The administration personnel are able to easily retreive information like "how many patients in the last five years have been diagnosed with Alzheimers?"
The Doctors are easily able to get trend analysis to questions like "how many patients died of the total diagnosed with Alzheimers in the past five years"

Users have extensive system generated reports with option of importing into Microsoft Word.

A Fully Customizable Alzheimer's Clinic Patient Tracking and Statistical application. The clinic can create a custom-category schema from the ground up, or work from one of Synergy's Alzheimers Clinic sample category sets consisting of 80 sub-sets.

Test scorings, questionnaires and statistical criteria can be customised to suite the specific clinics needs.

Relational Database technology giving automatic safeguards against duplicate record entry and the power and ease of retrieving information from the system by use of 3rd party reporting tools like MS-Query or directly from Microsoft Access.

Ability to integrate with other existing or new systems like scheduling and accounting.
